El Pla d’innovació d’atenció primària i salut comunitària (PIAPiSC) apareix com a conseqüència i evolució del nostre sistema sanitari i per la necessitat permanent d’adaptació a l’entorn canviant, per tal d’oferir la millor resposta possible als problemes de salut de la ciutadania. Amb aquesta finalitat, les propostes del Pla en els àmbits estratègic i operatiu (algunes d’aquestes sorgides en diferents territoris) volen facilitar progressivament la modernització dels serveis d’atenció primària.

The Primary Healthcare and Community Health Innovation Plan (PIAPiSC) is the result of the development of the Catalan healthcare syystem and the constant need to adappt to a changing environment, in order to offer the best possible response to the public’s health problems. Hence, the aim of the Plan’s proposals for strategic and operational areas (some of which have arisen in other regions) is to gradually facilitate the modernization of primary healthcare services.
