Abstract

Title: Listening to the heart of care: a qualitative phenomenological study to understand moral distress in nursing practice. Introduction: Moral distress represents a critical challenge to the mental health and professional performance of nurses, negatively affecting the quality of care and the sustainability of healthcare systems. This phenomenon, widely reported in diverse clinical contexts, has been experienced by a high percentage of professionals at some point in their careers. Aim: The aim of this study is to understand the phenomenon of moral distress in the daily practice of nurses in different settings, such as hospitals, primary care centres, mental health, social and healthcare centres and voluntary work, identifying the main factors that trigger it. Methodology: A qualitative methodology with a phenomenological approach will be used, suitable for exploring in depth the lived experience of the participants. Data collection will be carried out through semi-structured interviews and focus groups with nurses from different healthcare settings. The Atlas Ti software will be used to analyse the results obtained, following the criteria of scientific rigour of qualitative research: credibility, transferability, dependence and confirmability. The expected results of this study will allow a deeper understanding of moral distress in different contexts, contributing to the development of health policies aimed at the prevention and management of this phenomenon. They will also inform the design of interventions that promote the emotional well-being and resilience of nurses, strengthening their capacity to respond to ethical dilemmas and situations of conflicting values in their daily work.

Título: Escuchando el corazón del cuidado: un estudio fenomenológico cualitativo para comprender la angustia moral en la práctica de enfermería. Introducción: La angustia moral representa un desafío crítico para la salud mental y el desempeño profesional de las enfermeras, afectando negativamente la calidad del cuidado y la sostenibilidad de los sistemas sanitarios. Este fenómeno, ampliamente reportado en diversos contextos clínicos, ha sido vivido por un elevado porcentaje de profesionales en algun momento de su trayectoria. Objetivo: El objetivo de este estudio es comprender el fenómeno de la angustia moral en la práctica diaria de las enfermeras en diferentes ámbitos, como hospitales, centros de atención primaria, salud mental, centros sociosanitarios y voluntariado, identificando los principales factores que lo desencadenan. Metodología: Se empleará una metodología cualitativa de enfoque fenomenológico, adecuada para explorar en profundidad la experiencia vivida de las participantes. La recolección de datos se realizará mediante entrevistas semiestructuradas y grupos focales con enfermeras de diferentes ámbitos sanitarios. Para analizar los resultados obtenidos se utilizará el software Atlas Ti, siguiendo criterios de rigor científico de la investigación cualitativa: credibilidad, transferibilidad, dependencia y confirmabilidad. Los resultados esperados de este estudio permitirán una comprensión profunda de la angustia moral en contextos diferentes, contribuyendo al desarrollo de políticas de salud orientadas a la prevención y manejo de este fenómeno. Asimismo, podrán informar el diseño de intervenciones que promueven el bienestar emocional y la resiliencia de las enfermeras, fortaleciendo su capacidad de respuesta ante dilemas éticos y situaciones de conflicto de valores en su labor diaria.
