Los cuidados dignos al final de la vida desde la perspectiva enfermera: una revisión sistemática de la literatura

Introducción:
El interés sanitario de mantener la dignidad en los cuidados al final de la vida (FV), ha
promovido el cambio de un modelo biomédico tradicional a uno bio-psico-socio-espiritual, para
respetar la dignidad en todas las dimensiones de la persona.
La ausencia de conocimientos sobre cuáles son los mejores cuidados dignos al FV junto con el
número de enfermeras que se sienten poco preparadas, hace necesario profundizar sobre los
cuidados dignos al FV proporcionados por enfermería.
Objetivo:
Realizar una revisión sistemática de la literatura con el fin de identificar y sintetizar los
cuidados dignos al FV desde el punto de vista enfermero.
Metodología:
Se diseñó una estrategia de búsqueda combinando palabras clave, que posteriormente fue
aplicada en las bases de datos PubMed, CINAHL y Web of Science. Tras eliminar los artículos
duplicados, se obtuvieron 83 artículos relevantes por título, de los cuales 47 fueron incluidos
finalmente en la revisión.
Resultados:
A la luz de los estudios publicados, se identificaron 3 temas. El primero incluye a la persona y
se divide a su vez en 4 subtemas que coinciden con la dimensión física, emocional,
existencial/espiritual y social de la persona. Los otros dos temas identificados han sido la
relación enfermera-paciente y la planificación avanzada de los cuidados. De entre los subtemas
más relevantes se destacan: un buen manejo sintomatológico, proteger la privacidad del cuerpo
y del entorno, promover la autonomía, la comunicación, la escucha activa, el bienestar espiritual
y el confort físico, detectar las necesidades, mantener las relaciones significativas y asegurar la
atención continuada y el cuidado holístico del paciente.
Conclusiones:
Enfermería tiene un rol imprescindible en el cuidado digno al FV y la aplicación en la práctica
diaria de los cuidados dignos identificados en este estudio, ayudaría a humanizar y dignificar el
FV del paciente. Además, enfermería debe tener en cuenta las barreras identificadas, ya que
pueden proporcionar una percepción de pérdida de dignidad del paciente. |

Introduction:
The sanitary interest to maintain dignity in the care at the end of life (EoL), has promoted the
change of traditional biomedical model to a bio-psycho-socio-spiritual, to respect the dignity in
all dimensions of the person.
The lack of knowledge about which is the best care at the EoL and the number of nurses who
feel unprepared, makes it necessary to deepen the dignified care at the EoL provided by nursing.
Objective:
To carry out a systematic review of the literature in order to identify and synthesize the
dignified care at the EL from the nursing point of view.
Methodology:
A search strategy combining keywords was designed, which was applied on PubMed, CINAHL
and Web of Science databases.
After eliminating duplicate articles, 83 relevant articles were obtained by title, of which 47 were
finally included in the review.
Results:
From all the published studies, 3 themes were identified. The first one includes the person and is
divided into 4 subtopics coinciding with the person‟s physical, emotional, spiritual/existential
and social dimension. The other two themes identified include the relationship between the
nurse and the patient and the advanced care planning. Among the most relevant sub-themes
stand out: good symptomatic management, protect the privacy of the body and the environment,
promote autonomy, communication, active listening, spiritual well-being and physical comfort,
detect the needs, maintain meaningful relationships and ensure continued attention and holistic
care of the patient.
Conclusions:
Nursing has an essential role in the dignified care at the EoL and the application in daily
practice of the dignified care identified in this study, would help to humanize and dignify the
patient's EoL. In addition, nursing must take into account the barriers identified, since they can
provide a perception of loss of dignity of the patient |
