Importància del vincle terapèutic en el procés de derivació d’un pacient psicòtic d’inici precoç

Abstract

L’objectiu al descriure aquest cas és analitzar la importància del vincle terapèutic per tal de realitzar una derivació adequada a un servei d’adults un cop arribada la majoria d’edat d’un pacient psicòtic d’inici precoç. A més, es descriu la intervenció en rehabilitació cognitiva al llarg de dos mesos a les consultes externes de la Unitat de Trastorn Psicòtics Infanto-Juvenil de l’Hospital de Sant Joan de Déu. En la intervenció s’observa a un pacient amb un alt nivell de pensaments paranoides els quals es van reestructurant amb diverses tècniques i exercicis disminuint el seu malestar i millorant la seva qualitat de vida. Consisteix en una evolució lenta però satisfactòria si el pacient és constant. D’altra banda, el vincle terapèutic establert entre el pacient i el terapeuta és fort però dificulta la una derivació satisfactòria ja que la mare es mostra fins al final molt reticent al canvi.

The object of this analysis is to highlight the importance of the therapeutic connection in order to adequately assess early-onset psychosis in an adult patient recently into adulthood. The cognitive rehabilitation treamtent is detailed over a period of two months, recorded at the external consultations of the Childhood-Youth Psychoactive Disorder Unit of the Hospital de Sant Joan de Déu. It is observed that, during the symptomatology intensification intervention, a patient's paranoid thoughts are restructured using various techniques and excercises, further reducing their discomfort and improving their quality of life. The treatment contains a slow but satisfactory evolution, as long as the patient is consistent. Even though the therapeutic link between the patient and the therapist is strong, it did not result, however, into a satisfactory derivation for both parties; because the mother remained reluctant to adopt change.
