Introduction: Postpartum depression is a prevalent and often underdiagnosed mental health disorder that affects women during the first year after childbirth. This study examines its multifactorial etiology, clinical manifestations, risk factors, and the severe repercussions it has on the mother, the infant, and the family environment, emphasizing the importance of early diagnosis and appropriate intervention. Breastfeeding is recognized for its significance in the health of both the infant and the mother, providing optimal nutrition, protecting against diseases, and promoting cognitive development. Despite its benefits, the global prevalence of breastfeeding remains low due to various social, economic, and cultural factors influencing its practice. Objective: The primary objective of this project is to assess the association between breastfeeding and the risk of developing postpartum depression during the first 12 months. Secondary objectives include determining the incidence of postpartum depression in mothers who breastfeed compared to those who do not, identifying sociodemographic and clinical factors that may increase the risk of the condition, and evaluating the impact of the duration of breastfeeding on reducing the risk of developing the disorder. Methodology: It is a prospective cohort observational and analytical study to be conducted in the Girona Health Region, including Primary Care Centers (CAP) and Sexual and Reproductive Health Services (ASSIR). The study population will consist of mothers who meet the established inclusion and exclusion criteria. A sample of 498 participants will be recruited using consecutive non-probabilistic sampling. Data will be collected through a self-administered ad hoc questionnaire, which will include sociodemographic variables, breastfeeding practices, and mental health, as well as the Edinburgh Postnatal Depression Scale (EPDS) to assess depressive symptoms. Follow-up will be conducted at 0, 3, 6, 9, and 12 months postpartum
