Violència obstètrica: protocol d’infermeria centrat en el respecte al pla de part i a la dona

Introducció: Diferents estudis que remarquen la falta de consens sobre com definir la violència
obstètrica, tal com va declarar el Fons de Població de les Nacions Unides (UNFPA).(3, 4, 6) Encara que
la definició publicada a Veneçuela l’any 2007 dins la “Llei orgànica del dret de les dones a una vida
lliure de violència”(6) la defineix, de forma oficial com “l'apropiació del cos i dels processos
reproductius de la dona per part del personal sanitari, que s'expressa com un tractament
deshumanitzat, un abús de medicació i la conversió dels processos naturals en patològics, que
comporta la pèrdua d'autonomia i la capacitat de decidir lliurement sobre el seu cos i la seva
sexualitat, afectant negativament la qualitat de vida de les dones”(3). Aquesta situació és una
vulneració clara dels drets humans i de les dones i representa un problema greu per a la salut
pública, i per això és important la recerca de millores i mètodes de treball que puguin canviar
aquesta situació, la qual pot tenir conseqüències greus tant per la mare com pel nadó. (4, 7-10)
Objectiu: L’objectiu principal d’aquest treball és la creació d’un protocol d’infermeria basat en
l’evidència actual sobre l’atenció al part eutòcic que garanteixi que el procés sigui respectat,
humanitzat i atès de forma holística, donant llum al terme de violència obstètrica i tot allò que
implica.
Material i mètode: S'ha realitzat una revisió de la bibliografia actual basada en la recerca d'articles,
estudis, protocols i guies obtingudes de les bases de dades PubMed, CINAHL Plus, Scopus, Web of
Science, Dialnet i en varis centres hospitalaris. Per fer la recerca s'han utilitzat les paraules clau
mencionades a continuació i el cribratge ha acabat amb un resultat de 15 articles, reflectit en la taula
basada en el diagrama de prisma que es troba més endavant.
Resultats: Com a resultat s’ha obtingut un protocol d’infermeria basat en l’evidència actual sobre
l’atenció al part eutòcic que garanteixi una atenció respectada, humanitzada i de forma holística,
empoderant a la dona durant el procés.
Conclusió: Del treball se n’ha obtingut un protocol que té la intenció de servir de guia a les
infermeres per millorar l’atenció a la dona durant el part, evitant la violència obstètrica i garantint un
empoderament de la dona durant el procés estimulant la seva autonomia i participació.
Introduction: Different studies highlight the lack of consensus on how to define obstetric violence, as
stated by the United Nations Population Fund (UNFPA). (3, 4, 6) Although the definition published in
Venezuela in 2007 in the "Organic Law on the Right of Women to a Life Free of Violence"(6) officially
defines it as "the appropriation of the woman's body and reproductive processes by health
personnel, which is expressed as a dehumanised treatment, an abuse of medication and the
conversion of natural processes into pathological ones, which entails the loss of autonomy and the
ability to decide freely about their bodies and sexuality, negatively affecting women's quality of
life"(3). This situation is a clear violation of human and women's rights and represents a serious
problem for public health, which is why it is important to seek improvements and working methods
that can change this situation, which can have serious consequences for both mother and baby. (4,
7-10)
Objective: The main objective of this work is the creation of a nursing protocol based on current
evidence on the care of euthecological childbirth that guarantees that the process is respected,
humanised and cared for holistically, giving light to the term obstetric violence and all that it implies.
Material and methods: A review of the current literature was carried out based on a search of
articles, studies, protocols and guidelines obtained from the databases PubMed, CINAHL Plus,
Scopus, Web of Science, Dialnet and various hospital centres. The search was carried out using the
keywords mentioned below and the screening resulted in 15 articles, reflected in the table based on
the prism diagram below.
Results: As a result, a nursing protocol based on the current evidence on euthycotic birth care has
been obtained that guarantees a respectful, humanised and holistic care, empowering the woman
during the process.
Conclusion: A protocol has been obtained that is intended to serve as a guide for nurses to improve
care for women during childbirth, avoiding obstetric violence and guaranteeing the empowerment of
women during the process by stimulating their autonomy and participation.
