Recently, treatment options have been slim, with chemotherapy and, in certain cases, IL-2, the only available and recognized options. However, immunotherapy has emerged as a hopeful alternative, due to the discovery of the relationship between the tumor and the cells of the individual's immune system, which may be crucial for the course of the disease, showing substantial improvements in the survival of individuals with melanoma in advanced stages. Objective: Provide scientific evidence on the use of tumor-infiltrating lymphocyte therapy as a treatment for metastatic melanoma. Material and methods: An exhaustive bibliographic review has been carried out using the PubMed and Elsevier databases during the months of January to April 2024. The search has been limited to studies published in English between 2018 and 2024. After applying the criteria inclusion and exclusion, a total of 10 articles were selected for analysis in this study. Results: The selected studies support that tumor-infiltrating lymphocyte therapy shows response rates greater than 60% compared to other available T cell therapies. The therapy has also been observed to lead to an improvement in overall survival and disease progression. Conclusion: The results of the literature review support the effectiveness of treatment with tumor-infiltrating lymphocytes in patients with metastatic melanoma. However, some problems arise that require attention, such as the need for phase II and III clinical trials, improve the method and expansion time of TILs, as well as provide training to professionals involved in this therapy Introducción: En los últimos años, ha habido un incremento en la incidencia del melanoma cutáneo, lo que se ha traducido en un aumento de la mortalidad asociada a esta enfermedad. Recientemente, las posibilidades de tratamiento eran escasas, con la quimioterapia y, en ciertos casos, la interleucina-2 (IL-2), como las únicas opciones disponibles y reconocidas. No obstante, la inmunoterapia ha emergido como una alternativa esperanzadora, debido al hallazgo de la relación entre el tumor y las células del sistema inmunitario de la persona con esta afectación, pudiendo ser crucial para el curso de la enfermedad, evidenciando mejoras sustanciales en la supervivencia de personas con melanoma en fases avanzadas. Objetivo: Aportar evidencia científica sobre el uso de la terapia con linfocitos infiltrantes tumorales como tratamiento del melanoma metastásico. Materiales y métodos: Se ha llevado a cabo una exhaustiva revisión bibliográfica utilizando las bases de datos PubMed y Elsevier durante los meses de enero a abril de 2024. La búsqueda se ha limitado a estudios publicados en lengua inglesa entre los años 2018 y 2024. Después de aplicar los criterios de inclusión y exclusión, se seleccionaron un total de 10 artículos para su análisis en este estudio. Resultados: Los estudios obtenidos respaldan que la terapia con linfocitos infiltrantes de tumores muestran tasas de respuesta superiores al 60% en comparación con otras terapias con células T disponibles. También se ha observado que la terapia conduce a una mejora en la supervivencia general y en la evolución de la enfermedad. Conclusiones: Los resultados de la revisión bibliográfica respaldan la efectividad del tratamiento con linfocitos infiltrantes de tumores en personas con melanoma metastásico.
